Enrollment Guidelines and Procedures for incoming First Year Medical Students
For Filipino Accepted Applicants
STEP 1. Be ready with your Documents for Enrollment
Your Printed/Original Documents must be submitted to the Dean's Office through courier or by the time the university will be allowed to implement "Cautious return for in-campus face-to-face classes". FOR NOW, electronic files are allowed. Your documents in electronic form should be in PDF format. Each file must contain only one type of document (ex. Transcript of Records). As a convention, this is how to set the file name for each document: <LAST NAME>_<DOCUMENT>
Example: Transcript of Records of Juan de la Cruz, File name: DE LA CRUZ_TOR
Please take note of the following documents for submission in electronic form (PDF) and corresponding file name formats:
1. Original Transcript of Records (TOR issued for employment or board exam purposes will not be accepted).
File name: <LAST NAME>_TOR
2. Certification of Graduation with general weighted average (GWA/GPA/QPI) from the Registrar or Dean.
File name: <LAST NAME>_GWA
3. College Diploma or certificate of graduation if diploma is not yet available.
File name: <LAST NAME>_DIPLOMA
4. Certificates of Good Moral Character from (2) college professors (2 documents).
File name: <LAST NAME>_GMC1
File name: <LAST NAME>_GMC2
5. Clear Birth Certificate from PSA.
File name: <LAST NAME>_BIRTH
6. Transfer Credentials/Honorable Dismissal (for non-XU graduate), except for Universities that do not issue Transfer credential or honorable dismissal.
File name: <LAST NAME>_TC
7. NMAT Official Result. This is the printed NMAT result mailed to the admission applicant from the CEM. Please contact CEM if your printed NMAT result has not arrived.

8. Marriage Certificate, if married.
File name: <LAST NAME>_MARRIAGE
9. Recent ID picture (allowed sizes: 1x1, 2x2 or passport size)
File name: <LAST NAME>_ID
Documents may be scanned or taken through mobile device. Make sure that the image is CLEAR and contents are READABLE. Birth Certificates issued from the PSA should likewise be readable, otherwise, the enrollee shall be required to submit a readable birth certificate issued from the local civil registrar.
STEP 2. Pay the required Downpayment of ₱ 12,000
Before paying the downpayment, make sure that all documents in Step 1 are complete. Downpayments are accepted through any of the University's ACCREDITED PAYMENT CENTERS. Please see the list of our accredited payment centers through this link: https://www.xu.edu.ph/payment-centers-finance
Kindly follow diligently the instructions in filling out the bank forms or payments slips. Please take note of your REFERENCE NUMBER:
1. If you have been a student of Xavier University, your reference number is <YOUR CAMPUS ID NUMBER><LASTNAME>
2. If you are a NEW student of Xavier University, your reference number is provided in your acceptance letter.
Please scan or take a clear photo of your bank-validated deposit slip or bank transfer confirmation or payment slips validated by any of the accredited payment center, as it is required for submission in Step 3, along with the documents listed in Step 1.
STEP 3. Proceed to ONLINE ENROLLMENT and submission of Documents
To proceed with the enrollment process, please click on the button "Enroll Now" below to input your Personal Data in our Online Personal Data Sheet and submit your documents:
